Helpful Insurance Information

Newborns: Remember to add your newborn to your insurance as soon as possible within the first month. This does not occur automatically. Please notify us once this is complete and we will bill your insurance for accumulated charges from the first few months. You will receive a statement from us that may include charges that you are not responsible for until we receive verification that your newborn has been added to your insurance. If your insurance is an HMO or requires you to select a doctor, please remember to choose one of our doctors as your PCP (Primary Care Physician) to ensure timely payment of claims.

Maximum Benefits:  We participate in the VFC (Vaccines For Children) program through our health department, which provides us with free vaccines for those families whose insurance does not cover vaccines or who have met the maximum benefit.  Please call your insurance company to verify coverage.

 

Co pays:  Many insurance companies require co pays for office visits.  We expect you to pay your co- pay at the time of the visit.  We accept cash, checks, and Visa, MasterCard and American Express.

 

Deductibles:  Your insurance may have a deductible, an amount that must be paid by you before your insurance starts.  Please review your insurance benefits. You may not have to meet a deductible to have well child care covered.

 

Please bring your insurance card and identification with you to all office visits.

 

Please retain your receipts and explanation of benefits from your insurance company.  If a claim is rejected, please call your insurance company first.
